Coles County Telecare provides a wide variety of services to individuals 60 years of age and older such as:
Coordinated Point of Entry for Information, Assistance, and Referral Services: Telecare provides information, assistance, and referral services for persons 60 years of age and older. Services included “Benefits Counseling”, “Long-Term Support Options Counseling” using “Person-Centered Planning”, assessing client needs and providing advocacy to secure needed benefits and services, thus helping our clients to achieve an increased quality of life. Telecare also conducts follow-up activities to ensure the clients’ needs have been met.
Outreach Services: Telecare Staff will make home visits to homebound older adults through our “Info on the Go” mobile service. During home visits, persons over 60 are assessed for benefits and services available, and are assisted in gaining access to those benefits and services.
Completion of Forms and Applications including Benefits Access Applications: Telecare assists clients in completing and submitting various forms required to receive benefits from local, state, and federal programs. These programs include Medicare, Medicaid, Benefits Access Program (formerly Circuit Breaker), and many others. Telecare assists in Medicare Part D Prescription Plan review and enrollment and provides basic supplemental insurance information and referral as needed. Telecare participates in continuing education and training throughout the year including Disaster and Emergency Preparedness and information regarding changes as a result of the Affordable Healthcare Act.
Fiscal Year 2015 Coles County Telecare served 1426 older adults from Coles County and 118 from other surrounding counties. As a Senior Health Insurance Assistance Program, 462 applications for Benefits Access were submitted and 172 Medicare Part D Enrollments were completed.. No fees are associated with this program however; donations from clients and their loved ones are encouraged.
